How can we correctly code 20610 for bilateral knees and bilateral hips on same day? The insurance company (Humana) states it MUST be on ONE LINE ONLY.
Would 20610-50-51 with 1 unit, quadruple the price, work?

You could send 2 separate claims - just make sure the diagnosis clearly reflects the different locations. It might save time to send records with the claim(s) too.

Are you certain they stated different locations had to be on one line or did they state bilateral injections had to be on one line? I have never had an insurance state that injection on different areas have to be reported on one line, however it is common to have them require bilateral be reported on one line.
If you submit two claims one will be denied as a duplicate claim. I would use two lines one for the knees and one for the hips with the 50 modifiers on each and a 59 50 on the second and link the knee dx to one and the hip dx to the other.
