Our physician did a botox injection to the levator scapulae and subscapularis muscle for spasmodic torticollis and spasm.

From what I have researched, the levator scapulae is situated at the neck and back and is innervated by the spinal nerves C3, C4, and C5.

The subscapularis originates at the scapula and is a muscle of the rotator cuff.

Would the injection of the levator scapulae be described by CPT 64616- Chemodenervation; neck muscle(s), unilateral (eg, for cervical dystonia, spasmodic torticollis) due to the muscle location or CPT 64615 - Chemodenervation; muscle(s) innervated by facial, trigeminal, cervical spinal and accessory nerves, bilateral (eg, for chronic migraine) due to the innervation?

For the subscapularis I am thinking 64642: chemodenervation of 1 extremity; 1–4 muscles?
