Wiki need help with code 28675 and 28525

I have a modifier 59 and lt on both. They are being denied for missing modifier what other modifier do these codes need? please help

I would have used 51, dependent on payer-would need more info on the op note

Is the denial for a missing modifier or an invalid modifier? There is no CCI relationship between these two codes so unless there's a payer-specific bundling policy, I'm not sure I'd use a 59 modifier on either code.
