I work for an ENT and need to get pre-determination for the following surgery, I need help with codes:

Left Caldwell-Luc with biopsy intranasal mass, Septoplasty, Microdebrider assisted turbinoplasty with outfracture, Left Nasal Polypectomy, Left Intranasal Ethmoidectomy

I would appreciate the help with this.
