I have a situation where the patient is in hospice and his daughter called his physician to discuss the situation. The physician called the daughter back and basically told her (in not so many words) that the pt's condition is worsening and she should consider stopping his dialysis. The pt didn't participate in the call (he has Alzheimer's) but auths were signed so there's no issue there.

I'm mostly stumped on what DX code to use regarding the "counseling" since I've never had this come along before - when the family initiates the "counseling" or "conference". I'm open to any ideas.
