Wiki Need help with coding an ORIF for distal humeral fracture.

Incision made at lateral aspect of the distal upper arm, dissected down to the fracture site. Fracture site was reduced. Multiple cables were wrapped around the oblique fracture site. Tow screws inserted to compress the fracture and prevent rotation. Then radial nerve was identified and protected. Surgical site was closed layered manner.
Not sure if anyone can help. Keep going to 24546. I do not have enough information to support the code.
New to this only a few months with Ortho.

Are you able to redact and put the whole op note with headers? There is not enough information here.

This is one of my pet peeves when reading op notes. The provider went to the "fracture site". Where is the fracture site? Without specific documentation you can't code. Is the fracture of the shaft, supracondylar or epicondylar? All of those will change the coding. Hopefully the specific location is documented somewhere in the note.
