Wiki NEED HELP WITH CODING SEQUELAS FROM STROKE/CVA

We get alot of patients who follow up for having a CVA/stroke and their only sequela is right/left side weakness. That is exactly how the providers document it, I had read in some articles that in the note it HAS to say hemiplegia/hemiparesis/monoplegia (ex: i69.331,i69.351) and if it is just documented "weakness" than were supposed to code it as z86.73 & r53.1 together. But i've read other conflicting articles, can someone please advise me on how to correctly code this ?
