Need help with CPT and diagnosis.

op note reads: The skin was excised in an elliptical fashion through the full thickness of the skin down to healthy appearing subcutaneous fatty tissue. The lesion was removed and passed off the field for permanent pathology. Hemostasis was achieved with electrocautery as needed. The skin edges were undermined with cautery and then approximated with three interrupted sutures of 5-0 plain gut.
 
To code a lesion removal you must have the size of the lesion and the pathological results to ascertain what type of lesion it is.
Lesion codes are based on the size of the removed the lesion, where it was removed from, and whether or not it was benign or malignant.
