Need some help and opinions for a new procedure my docs are performing:

proc: suction assisted lipectomy
md performs suction-assisted lipectomy to bring dialysis av fistula closer to skin level
due to the patient having large arms. Doing this would make access to the av fistula easier to provide dialysis treatment.

thought of cpt 15878, suction-assist lipectomy (maybe not get pd since not cosmetic)?
thought of cpt 36832, revision of fistula/graft (does not seem to fit the procedure)?
thought of unlisted cpt?

then what kind of dx would you use? for large arms....
