It depends if it is malignant, benign, or unspecified.

the 403.90 code instructs you to add the 585.- code for the stage of the CKD
ICD-10 CM code is I12.9 plus the N18.- code for the stage of the CKD
for the diabetes and Nephropathy the provider must document a link for these conditions such as diabetic nephropathy or nephropathy due to diabetes they you may code the 250.40-250.43 depending on the type of diabetes, plus the code for the nephropathy. If not documented as linked together then code only the diabetes as 250.00-250.03 and code the nephropathy.
The ICD-10 CM codes for the Diabetic nephropathy is E11.21 for type 2 diabetes , E10.21 for type 1
