The Hcpcs Code Q0138 is being denied for the reason that relative Dx code is missing even though I used one Dx code of D50.
Then I did google to find out where I am wrong. One of website says this hcpcs code requires two dx code to be paid and they gave reference of one website but that website says about ICD 9 condition.
Does this condition of using two dx codes still apply with this hcpcs code or not. Any comments or reference please, thanks

You will need to use two dx codes. They should reflect the type of anemia the patient has and the level of chronic kidney failure.
