Can anyone please help me with an icd-9 code for a history of an Intracranial Aneurysm Repair? The patient had an MRI Brain and the findings were no Significant abnormalities on MRI Brain. The DX on the Dr's Script were Common Migrane and a histroy of an Intracranial Aneurysm. The Insurance is Medicare so the claim was denied as not medically necessary. Thanks
 
utilizing 3M coder I came up with the following:

V12.59 persoanl history of disease of the circulatory system followed by V67.09 follow-up exam following other surgery
 
That is good. But have a look at this code also because it comes closer to the point of having had some surgery in the vessels, than simply describing diseases of the circulatory system.
V15.1 Surgery to heart an dgreat vessels- the great vessels need not necessarily apply to heart vessels alone.It is surgery in other vessels too
