Can anyone help with a ICD9 for otitis conjunctivitis.

Are you sure this isn't a mistake? I would show the doc the ICD-9 book the page with otitis and also the page with conjunctivitis and have him/her pick the code based on the descriptions in the book.
 
Perhaps the patient has an otitis and conjunctivitis? You need to know the type of otitis (ie media). I would check the chief complaint and then take the note and the charge slip to the provider for clarification.
