Pt. had Rt Salpingectomy, Rt Paratubular Cystectomy and Exploratory Laparotomy. I have it as 58700 and 49000,51 but I'm having some trouble finding a code for the paratubular cystectomy. Any help would be greatly appreciated.....
 
The 49000 is included in and will bundle with the 58700 (no modifier allowed). There isn't a specific code for the paratubal cystectomy. If I was coding for this procedure alone, I'd use one from the 49203-49205 range, depending on the size, but you can't bill a 49203 with the 58700 - the 58700 will bundle in with the 49203 (no modifier allowed).

In this case the cystectomy is on the same side as the salpingectomy, and I'm pretty certain any carrier is going to consider it incidental to the salpingectomy. If your physician spent a significant amount of extra time & effort on the cystectomy, your best choice is to put a -22 on the 58700 & make a case for extra $$.
