Hello,


If the Medicare guidelines states they will only cover injection of 2 levels bilaterally per session and my physician is adamant that he should do 3 levels bilaterally, can we have the patient sign an ABN and pay for the 3rd level? Or should we not bill that level at all to anyone?

Any guidance on this issue is greatly appreciated.
