PER SURGERY OP REPORT::The patient had a very large right multinodular goiter which required dividing the strap muscles on the right-hand side to gain access to the lateral aspect of the gland using the harmonic scalpel. Suture ligatures were placed on the upper strap muscles forretraction. Then dissecting in the fascial plane along the right lobe of the thyroid the superior polevessels were dissected out and divided using harmonic scalpel. Then inferiorly in the middle thyroid vein and the inferior pole vessels were dissected out and divided using harmonic scalpel taking careto preserve the inferior parathyroid along with his vasculature. The superior parathyroid was also identified and preserved along with this vasculature. The recurrent laryngeal nerve was identified inthe tracheoesophageal groove and confirmed using the Nims stimulator and then followed superiorly up to its insertion in the cricothyroid notch. The thyroid was reflected medially and the Berry's ligament was released using harmonic scalpel and then the thyroid was dissected off of the anteriortracheal wall and divided just to the left of midline using the harmonic scalpel. The medial aspect ofthe left lobe of the thyroid was dissected away from the trachea and removed en bloc with the rightlobe of the thyroid using the harmonic scalpel.
