Physician places an implantable VAD device and creates a neo-pericardium for reentry purposes (for easier access at time of transplant). He wants to bill separately for the neo-pericardium creation, but wouldn't it be included, since it is the "technique" that he uses? If any of you have any referrence sites that I can use to explain this to him, please let me know.
