Pt previously underwent laparoscopic partial nephrectomy. now is having open radical nephrectomy (same kidney-RT)
pathology shows renal cell carcinoma with invasion into the renal sinus fat..recurrent tumor with a tumor vein thrombus

Would this be a partial nephrectomy since there already was a partial done ?

Confused..thanks for any thoughts/help

Hi. No, I would code this as a radical nephrectomy, 50230-RT. When a partial is done, they are only taking a piece of the kidney with usually a tumor. When a radical is done, they take the kidney with gerotas fascia, perinephric fat, small piece of the ureter, sometimes the adrenal gland, and regional lymph nodes. If the path report supports the removal of all the “extras” along with the kidney, a radical is supported.
