My Dr is asking :

what are the circumstances under which 64421 can be used? We do this all the time on most of our patients and I haven’t been coding it; but I’ve been told by other surgeons that that is not correct. In other words it should be able to be coded. Are there circumstances under which it can be coded? Does it matter whether or not you put in any type of catheter device, such as” on Q” Or other device?

This bundles with 32480 & 32505. Anyone ever code the nerve block at all?
 
One of my surgeons asked the same thing recently and from what my manager and I found was it is considered part of the surgery if the surgeon performs the nerve block. However the anesthesiologist can bill if he/she performs this.
