If my provider performs motor and sensory studies on the distal Median nerve and on the proximal Median nerve, do I count 2 or 4 tests for coding? I know 4 tests were done but it is on the same nerve.

If the distal and proximal points were on the same branch of a nerve, then you can't count them separate; these are counted as the same nerve, and can only be counted as 1 test. Based on what you stated, I would code 95907--1 test Motor median nerve, 1 test sensory median nerve. See Appendix J in the CPT book--anything that is listed separately can be counted as 1 test.
