Neuro vs. ortho billing new pt visits

I just got an email from one of our neuro doc's wanting to know what percentage of his practice has to be neuro before he can bill new pt visits for a patient that has already been seen by one of our general ortho surgeons. His taxonomy is set up as neuro, but most of his practice is spine/ortho. I cannot find the answer anywhere.
Any help would be great!

That's what we originally thought but Medicare has been denying these. I even called medicare and I don't think that they were listening to what I was saying because they just told me that I need to call provider enrollment and change his specialty.:confused:
