Per the AANS, they recommend coding the Aspen Posterior Spinous Process Device with a 22841 and say it is not a stand alone type of instrumentation. When pedicle screws are also placed and I would normally code a 22840 for a one level, do I need to append a modifier to one or the other (22840 & 22841)?
 
I haven't had an issue billing instrumentation codes without a modifier; except for 22851. I just had a huge surgery involving these two exact codes. I did not add modifiers; however, that's not to say that the carrier won't be fussy and need records after the fact.
