1. Can You Charge For Removal And Placement Of Instrumentation When You Take Out A Plate And Put A New Plate In? Not Reinsertion Of The Same Instrumentation. Example The Doctor Removed A Plate Form C5-c6 And Put A New Plate In. I Need Documentation To Support.

2. Can You Charge 22310 If You Do Not Physically Put The Collar On, But Just Order The Collar To Be Put On? Does Mean You Treated The Fracture Without Manipulation? I Need Documentation To Support.
