Have a Dr that is treating patient for trigger point injections, patient comes in for just a follow up from previous TPI. Patient states pain is worse then before. So the Dr elects to go ahead with another TPI even though one was not scheduled during this visit. Can the Dr get the E/M visit as well as the injection with modifier 25?
 
Because it was not a planned thing AND the Physician had to evaluate and determine whether it was needed during this visit...yes, absolutely add -25 to EM + trigger point injection. :)
