Would I use 93458 when a left heart cath is done with injections into coronary and LV?
And would I use it for a Left heart cath and only injections of the coronary?
 
The same code. 93458 says including intraprocedural injections for left ventriculography, when performed. The when performed means that if you do it fine if you don't fine. not extra money either way.
