Any advice for determining proper codes for excision of lesions would be greatly appreciated. I don't know why but determining the sizes to append the proper code is confusing to me.

Any help is appreciated.
 
The operative report needs to contain the length of the lesion removed and the length of the closure. You should have a discussion with your doctors if they are not including this in the body of the report. One of my docs actually dictates both of these numbers in the top portion of the op report which makes coding a breeze!
