I have read a lot about these codes and my question is; Does the doctor have to state the time spent "on the phone" in the documentation? The way I read it, they are to "calculate" their time and pick the appropriate code. It doesn't say anything about stating the time spent in the actual documentation? Anyone?
 
I train my doctors to document the date and time they start and finish any type work, be it phone calls, exams, consults, or whatever. Once it is part of their routine, they don't even think about it, and once documented, it becomes fact.
