HELP! Is anyone out there using this code? How do we bill? I need to know what date we use or is it a date range. What is the Place of Service? What diagnosis? I'm assuming we use the surgery diagnosis. Is this done before surgery or after? I'm assuming that the manufacturer is billing for the device (98977) or does the provider bill for that?

Hello, I am also interested, a provider is wanting to begin billing for the first time. This would be for a physical therapist in private practice setting. Thank you
