Hello, I'm a bit confused over billing new patient visit with procedure. In particular, 99203-25 and 65222-RT have been paid by an insurer but I had thought that if a patient comes in with a problem already known (the foreign body in eye), doctor can not code both visit and procedure? Or am I incorrect on this?
