If he is being seen by the same provider or another provider of the same specialty from the same group, he is established. For example, if a cardiologist sees the patient inpatient and then follows up with the patient in his office, the office visit is established. If the patient is seeing another doctor in the group with a different specialty (such as a general surgeon) and who he did not see inpatient, then that would be a new patient visit.
