we saw a patient for the 1st time for a sick visit but the provider coded it as 99214 and we saw the patient a couple more times after that but never had a new patient appointment. can i bill 99204 after we saw the patient two times in the office?

No...the patient would be considered established for the subsequent visits. Depending how far back the original service took place, you may be able to send a corrected claim and report a new E/M level for the 1st visit. May be more trouble than it's worth.
