Wiki New patient E/M level of service

Doctor documented History (Detailed) , Exam (Expanded problem focused) and Medical decision making (Low) .for New patient.

As per my understanding it is (99202). But other coder's opinion is 3 keys are required and documented .Out of 3, 2keys meet with 99203. It is OK to code 99203.

I would like to get feedback from others with explanation .

For a new patient, all three components must be met, not just documented, so you are right 99202 is correct.
