Our PA had a new patient walk out. Here's what she said:
During the middle of the evaluation he walked out secondary to being confused as to why he was referred to us since we were not a pain clinic and that he was not interested in anymore surgeries. I had gone thru the NP packet with him, gotten his vitals and went thru the HPI and PMH/PSH stuff with him and was about to examine him when he became upset and left.

I just went back and re-read this and I'm getting clarification on what she means by "HPI, PMH/PSH stuff." I still welcome any input!

My initial instinct is that there is no criteria met, so no NP visit. He's Medicare/Medicaid, by the way.

I appreciate your input.

Additional info:

This is what I put in his chart:
HPI= history of present illness
PMH= past medical history
Allergies
Medication list
FH=family history
SH= social history
PSH= past surgical history
I only got to the vitals in the PE (physical exam) so I did not examine him. He has a TBI (traumatic brain injury) and has difficulty remembering things and was confused to why he was here. He may be back so I wanted to chart the history and that the patient left because he was a consult by a NP here.
