Is a new patient visit billable for a patient who transfers into care late into her already established pregnancy? For example, if a patient moves and has to establish care with an entirely new practice and she is already 20 weeks along in the pregnancy. This is not a confirmation of pregnancy as it already established, so would you bill the appropriate E/M NP code (99202-99205) with a routine prenatal visit code of Z34.02/Z34.82, Z3A.20? Or would this np establishing visit just be counted in with her antenatal visits?

Unless the visit was for care not related to the pregnancy, this would be part of her prenatal care. As the patient transferred in, you won't be able to bill global maternity. Assuming you provide all care moving forward, once she delivers you will bill:
Delivery + postpartum care 59410, 59515, 59614 or 59622 depending on the type of delivery
Antepartum care: if 1-3 visits, each individual visit. If 4-6 visits, 59425. If 7 or more visits, 59426.
