If a patient had a stress test in our office - with physician supervision required - the physician sees the patient at a later date - is the stress test because of supervision considered professional services by the physician - and now the patient is now considered established?
 
We bill an established visit for those patients. We had tried to bill a new patient visit, but our claims were denied. And looking back, it makes sense because the physician did have a face to face with the patient during the stress test. HTH :)
 
NP visit with previous stress test

That is what I find. I am trying to find reference to this from CMS. New patient description always talks about "face to face" by physician. Direct supervision requires physician to be readily available but not face to face.
