We are running into problems with a new provider that wants to bill a new patient E & M code and a procedure on the same day with a 25 modifier. We do know that a history and exam are included in the procedures and question if there is enough to bill an additional E & M code. Is there anyone that knows of a specific resource that would give us more definition on this for the specialty of Podiatry.
