We want to start billing for anesthesia in-house and have not previously. We don't fully understand the conversion factor rates for CMS or how to calculate it. Could anyone give me an example of what we would submit on a claim for 00142 if the anesthesia time was 15 min, pt is P1, and we are billing to CMS? Can we have a set fee, or do we have to calculate with the CF and submit the exact amount?
