just new at this! So if diagnosis of E11.29 and I10 are used does there need to be a diangosis code to identify state of chronic kidney disease and Long Term use of oral medication ? IE: Z79.84
 
if the patient has diabetes and chronic kidney disease then you would not use E11.29. You use E11.22 When you look in the tabular section of the code book you see that E11.22 instructs you to add a code for the chronic kidney disease. May I ask why you chose E11.29? Also if the patient is documented with hypertension as well then you would not use the I10 code. You use the combination code for Hypertensive chronic kidney disease.
