I am new to laboratory billing and have a couple questions.

1. Our in house lab is doing semi-quantitative drug testing, will I bill each drug separately to commercial insurances and G0434 to Medicare?

2. If the qualitative test was done in the office on the same day, do I bill 80104 to commercial insurances and G0431 to Medicare?

3. Can I bill both the qualitative and quantitative codes above together?

Any information on lab billing would be greatly appreciated.
