I work at an FQHC and we are still using our lots of old formulation in our vaccines for example 90734 Menectra; the new code has formulation quadrivalent, previously it was tetravalent. It is still the same code but we are using the tetravalent. We are also still giving 90645 Hib vaccination which has been deleted and replaced by 90648 which has a slightly different formulation. We have had denials for the deleted code 90645 that we are replacing now with 90648. Could we get written up for billing with the wrong formulation? Can someone research this and advise us on how to proceed.
