Wiki new versus established in multi-specialty clinics

I work in a multi-specialty practice of Fam Med, OBGYN, OCC Med, Walk-in-clinic opened 7 days-a-week, Rheumatology. If a patient is seen in the walk-in-clinic as a new patient and this patient is needing to be seen in Family Medicine, would this patient be an established or a new patient since Fam Med is a different clinic? Would this be the same in all the clinics.
 
If the TIN (tax ID) is the same, established pt EM.
If the clinics have different TIN (tax ID), then New EM if documented is okay.
