When a provider changes practices in the same specialty and the patients follow they are considered established patients but are they considered new or established if another provider of that new practice sees them? Thank you!

You're correct in that they are established to the original provider, even when seen in a different practice. If the other provider sees them before the original provider (in the new practice), then they are new to the other provider. Once the original provider sees them in the new practice as their established physician, any subsequent visits with any of the other providers are established as well.
