Wiki New vs. Established patient

I work in a hospital and recently started e/m coding for the family care centers. I'm a little confused about the new vs established pt rule. If a patient is treating with one of our cardiologists and is referred to one of our urologists, does that initial urology visit have to be a "new" patient or can we bill this as an established patient?

For urologist its a new patient because he is being referred to different specialty and that Doc is seeing the patient for first time.
