new wound care codes 2011

Does anyone know if the new wound care codes 97597 and 97598 are exclusive to POS in a wound care center? Are these for chronic wounds only or can you start out with 11042 for a certain number of treatment dates? (and then switch over to these codes?)

Is the 20 sq cm per anatomical site? ie right and left? or 20 sq cm for total bilateral?

We would also pose this question to 11042 and 11045. Any help is appreciated.
