One of our physicians saw a newborn in the hospital as a subsequent visit. After 48 hours child still had no stool; child spitting up and abdomen distended. Physician spent 1 1/2 hours with patient and was finally transported to Childrens Hospital. Please let me know what codes you would use for the additional time doctor spent with child.

Was what the actual diagnosis? If the baby was unstable/critically ill, you can use the critical care code 99291. (billed alone)

Otherwise you could use a "prolonged service code". Look at 99356. (billed with the hosp visit code)

Well that diagnosis may not support critical care....usually it is some sort of distress/failure... I'm assuming the baby was unstable if the doctor stayed by his side for 90 minutes...you are gonna have to check the chart notes or ask the doc.
