Patient comes in for nb weight check, they are dx with jaundice or nb feeding difficulties and told to return in 3 days for recheck. Would you code the 1st visit as sick and the return visit as well if problems have resolved? or Would you code the 1st visit as well with the additional dx of jaundice and feeding difficulties, and the return visit as sick? I don't think we should code both as well because you wouldn't do a well visit 3 days apart. Anyone who can give me some guidance on this I would greatly appreciate.

I would code the first visit with jaundice or nb feeding difficulties. It doesn't matter what the visit was scheduled for it is what was determined at the visit. So this would not be a wellness and if they are coming back to follow up on the jaundice or nb feeding difficulties then that as well would not be a wellness visit. This is my opinion and how we do it at my practice.:p
