In a radical prostatectomy, only 3 lymph nodes were excised.
1. Would these 3 nodes be billed individually as 88305-26 x3 or as a regional resection 88307-26?
2. Would the specimen below be coded as a lymph node (88305) even though there is no lymph tissue found?
3. Do I get to report 88309-26 x2 for pieces of the bladder neck and re-excision of the margin? Or is it included in the radical prostate surgical pathology/ Thanks!
C. Received in a formalin-filled container labeled with patient information and "periprostatic lymph node," are multiple pale tan irregular adipose tissue fragments measuring 2.5 x 2.0 x 1.0 cm in aggregate. No palpable lymph nodes are present in the tissue. The specimen is entirely submitted in cassette C1-C2.
C. Lymph node, periprostatic, excision:
Unremarkable adipose tissue.
Lymph node not identified.
E. Received in a formalin-filled container, labeled with patient information and "bladder neck final margin," is a single fragment of yellow-tan fibrous tissue measuring 0.4 x 0 cm. The specimen is inked black externally, trisected, and representatively submitted in one cassette.
F. Received in a formalin-filled container, labeled with patient information and "bladder neck additional margin," is a single fragment of yellow-tan fibrous tissue measuring 3.0 x 1.0 x 0.8 cm. The specimen is inked black at the resection margin, serially sectioned, and entirely submitted in cassette F1.
E. Final margin, bladder neck, excision:
Unremarkable fibroadipose tissue.
Negative for malignancy.
F. Additional margin, bladder neck, excision:
Unremarkable fibroadipose tissue and skeletal muscle.
Negative for malignancy.
