How would I code a Non-Surgical, Non-Healing wound?

Patient has a history of hypothyroidism and Hypertension, but is not diabetic. The open injury was sustained when a car door was slammed on the lower extremity 5 months ago.
 
Still more information is needed. Was the injury repaired initially, or was it left open due to contamination/infection? Different scenarios required different coding. The same injury can having radically different coding depending on the different types of circumstances and complications. This might be a continuation of the injury, it might be a complication. There is no way to assist without the complete scenario.
