Here is what I have to work with. Argh!

A transverse incision was reopened. A sinus tract was followed down to mesh which was removed. After thoroughly irrigating the area and noting no bleeding, 3 liters of ascites were removed from the peritoneal cavity. The soft tissue was approximated with Vicryl and the wound VAC was placed.

That's the entire thing. What do you think?
