On the notice of privacy practices is it required to have the entity's legal name in full?

i.e., Do you have to put the LLC at the end of the entity's name?

And also, does it have to be the entity's legal name or can it be the DBA (doing business as) name.

Nothing in the Privacy Rule or the Omnibus Rule answers this question. It sounds more like one for legal counsel and how they display the facility name on documents.
